Plain-language summary. This website is a marketing site. It collects only what you type into our forms (name, work email, practice name and your message) and standard website analytics. It is not designed to receive, store or process patient information, and you should never enter protected health information (PHI) here. We do not sell your information.
1. Who we are

2. Information we collect
- Form submissions. When you request a practice assessment or send us a message, we collect the fields you complete: name, business email, phone (optional), practice name, practice size, area of interest and your message.
- Website usage. Our hosting provider (Squarespace) collects standard technical data such as IP address, browser type, pages viewed and referring site, and may set cookies required for the site to function and for aggregate analytics.
- Email correspondence. If you email us, we keep the correspondence to respond to you.
3. What this website is not for
This public website is not a patient portal and is not a channel for clinical communication. Do not submit patient names, dates of birth, medical records, claim numbers, insurance identifiers, tax identification numbers, bank details or passwords through any form on this site. If such information is submitted, we will delete it on discovery. Any engagement that involves PHI is governed by a separate written agreement, including a Business Associate Agreement where required by HIPAA.
4. How we use information
- To respond to your request, schedule an assessment and follow up about SmartCareXI services.
- To operate, secure and improve the website.
- To send occasional product or event updates if you asked for them. You can opt out at any time by replying "unsubscribe" or emailing us.
5. Sharing
We share information only with service providers who help us run the website and respond to enquiries (for example, our website host and email provider), under agreements that restrict their use of it; when required by law; or with your direction. We do not sell personal information and we do not share it with advertisers.
6. Retention
Form submissions and correspondence are kept for as long as needed to respond to you and manage our relationship, and then deleted or anonymised. You can ask us to delete your information at any time.
7. Your choices and rights
You may request access to, correction of, or deletion of the personal information we hold about you by emailing info@smartcarexi.com. Illinois residents and residents of other states with privacy laws may have additional rights, which we will honour as applicable.
8. Security
The website is served over HTTPS and hosted by Squarespace. No method of transmission or storage is completely secure; please keep sensitive information out of this site as described above.
9. Children
This website is intended for healthcare professionals and practice owners. We do not knowingly collect information from anyone under 18.
10. Changes
We may update this policy; the effective date above will change when we do. Continued use of the site after a change means you accept the updated policy.
